Description Job Summary: Primary function is to provide personal healthcare and related services to the client in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse. Job Qualifications: Licensure: Must Be a Certified Nurse Aide in Texas | Texas Health and Human Services Experience: At least one year experience, preferred, as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency, or completion of Certified Nursing Assistant or Certified Home Health Aide Program. Skills: Must be able to read and write in English and follow written and verbal instructions in English effectively. Demonstrates interest in the welfare of ill and elderly. Proof of current CPR, and Hepatitis profile. Aide Competency Evaluation Skills Checklist and written Skills Test. Transportation: Reliable transportation and valid driver's license and auto liability insurance. Environmental and Working Conditions: Works in patient’s homes in various conditions, possible exposure to blood and body fluids and infectious diseases; the ability to work a flexible schedule, the ability to travel locally; some exposure to unpleasant weather. Physical and Mental Effort: Prolonged standing and walking required. The ability to lift up to 50 pounds and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and meet patient/family psycho social needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home. Qualifications Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy. Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. Practice accepted infection control principles. Provide a clean, safe and comfortable environment. Promote positive, supportive, respectful communication to patient/family and other employees. Provide an environment which promotes respect for patient, privacy and property. Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake. Contribute to the management and efficient operation of the agency and demonstrate effective time management skills. Provide skills necessary to perform treatments and procedures according to agency policy. Demonstrate commitment, professional growth and competency by attending 12 hours of in-services per year. Promote the agency philosophy and administrative policies to ensure quality of care.

Throughout the shift, you will assist with meals, escort residents to activities, monitor for changes in behavior or health, and document everything in the care log. Vital signs checks happen at regular intervals, and you will be the first person to notice if something seems off with a resident.

The team dynamic is close-knit. You will work alongside other aides, report to the charge nurse, and communicate with families who visit. In memory care units, redirection and engagement activities are a big part of the role — keeping residents calm, oriented, and socially active. The work is physically and emotionally demanding, but many aides describe deep bonds with the residents they care for and a sense of purpose that keeps them in the field long-term.
