CareBuilders at Home – Grapevine-Southlake TX - Job description: Job Title Commission-Based Healthcare Referral Representative Home Care and Senior Placement Services Job Description CareBuilders at Home is looking for a motivated and well-connected Referral Representative to help grow our private-duty home care and senior placement business. This is an ideal role for someone who already understands the post-acute care world and has experience or relationships within: Hospital discharge planning teams Skilled Nursing Facilities (SNFs) Rehab centers Assisted living and memory care communities Home health or hospice settings Case management or social work environments We are seeking someone who can open doors, build trusted relationships, and help generate qualified referrals for families in need of: Non-medical home care Companion care Post-discharge support Senior living placement assistance Compensation This is a commission-only role . You will be compensated based on the business you generate, including: Home care client starts Ongoing care hours Senior placement conversions Compensation structure will be discussed with serious candidates based on experience, access, and expected volume. What We’re Looking For The ideal candidate may be: Anyone who has proven success with revenue generation in Home Care Industry A former or current discharge planner, case manager, liaison, social worker, admissions coordinator, marketer, or other facility-based professional Someone with a strong network across hospitals, SNFs, rehab centers, or senior living communities Comfortable introducing CareBuilders at Home to families and care teams Able to identify opportunities where families need home care or placement support Professional, trustworthy, and relationship-driven Responsibilities Develop and maintain referral relationships in hospitals, SNFs, rehab centers, and senior care communities Identify potential client and family needs during transition points Introduce CareBuilders at Home services in a professional and compliant manner Generate qualified referrals that convert into home care or placement business Maintain communication with leadership regarding pipeline and opportunities Preferred Qualifications Experience in healthcare, senior care, discharge planning, case management, facility admissions, or post-acute outreach Existing relationships in the DFW healthcare or senior care market preferred Strong communication and interpersonal skills Self-starter with ability to work independently Proven success with revenue generation in Home Care Industry Understanding of patient discharge flow and family decision-making is a major plus Why This Role Can Be Attractive Flexible schedule No cap on earnings Opportunity to monetize your network and industry knowledge Ability to work with a growing home care and placement business Meaningful work helping families during stressful care transitions About Us CareBuilders at Home provides non-medical home care and senior placement services for families who need support after discharge, during recovery, or while navigating longer-term care decisions. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings.
