This position serves as a key resource for physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral sources by providing education regarding hospice services and benefits. The Hospice Clinical Liaison plays a critical role in expanding market presence, strengthening referral networks, and supporting organizational growth. Qualifications Qualifications: Minimum of two years sales and marketing experience in a healthcare setting. Thorough understanding of the hospice, home health, post-acute care, and healthcare referral market. Proven history of achieving or exceeding established admission, referral, census growth, or business development goals. Strong relationship-building, networking, and account management skills. Excellent communication, documentation, and organizational skills. Ability to develop and maintain professional relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral sources. Experience with hospice, home health, physician relations, community outreach, referral development, or healthcare business development preferred. Working knowledge of Microsoft Office applications. Ability to travel throughout the assigned territory. Ability to read, write, and speak English. Essential Functions: Meet or exceed admission, referral, census growth, and business development goals within the assigned territory. Schedule and execute a minimum of 40 sales calls per week, including outreach to new referral sources. Develop and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral sources. Educate referral partners, patients, families, and community stakeholders regarding hospice services and benefits. Identify opportunities to increase market presence, referral volume, and organizational growth. Communicate referral activity and market intelligence to leadership. Maintain accurate referral source logs and sales activity documentation. Prepare and submit weekly business development reports. Facilitate strategic physician and healthcare partner relationships. Represent the organization at community and healthcare networking events.

Maintenance staff handle a daily work order queue β€” everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming β€” exercise classes, crafts, music sessions, outings β€” and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
