The Staff Pad is proud to partner with a leading hospice and palliative care provider in the DFW Metroplex,to hire a compassionate Master Social Worker (MSW/LMSW/LCSW). Reporting to the Director of Family Care Services, The Master Social Worker provides coordinated hospice care by conducting psychosocial assessments, counseling patients and families, coordinating community resources, and supporting emotional, financial, environmental, and end-of-life needs. Schedule: Full-Time What You'll Do Key Responsibilities Conduct psychosocial assessments to identify patients' and families' emotional, social, financial, environmental, and resource needs Provide counseling and emotional support to patients and families navigating grief, loss, and end-of-life care Educate and facilitate advance care planning discussions with patients and families Coordinate community resources, referrals and support services to meet patient and family needs Develop and support individualized care plans Collaborate with physicians, nurses, chaplains, and the interdisciplinary team Maintain accurate and timely clinical documentation Communicate effectively with patients, families, and healthcare providers Participate in interdisciplinary team meetings and collaborate on patient care planning Maintain required licensure, continuing education, and professional competency Maintain knowledge of community resources and public/private assistance programs Manage clinical assignments within scope of education, licensure, and competency Promote a culture of compassionate, patient-centered care aligned with agency values and policies What We Are Looking For Qualifications Master's Degree in Social Work (MSW) required Texas LMSW or LCSW license required Minimum one year of healthcare social work experience Hospice, palliative care, home health, or related healthcare experience preferred Valid Texas driver's license, reliable transportation, and auto insurance required Additional Skills & Requirements Emotionally resilient, collaborative, and empathetic Strong communication, counseling, and documentation skills Skilled in psychosocial assessment and resource coordination Critical thinking and problem-solving abilities Successful background check Proof of TB testing and Hepatitis acceptance or declination What You Can Expect Benefits & Perks Competitive salary and mileage reimbursement Comprehensive medical, dental, vision, and short term/long term insurance 403(b) retirement plan Generous paid time off, 6 paid holidays, flexible days Education Reimbursement Assistance Program Schedule Flexibility & Professional development opportunities Supportive, collaborative team environment Meaningful work that makes a difference every day If you're passionate about supporting patients and families through life's most meaningful moments, apply today and join a team dedicated to providing exceptional hospice care with dignity, compassion, and support.

Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming β€” exercise classes, crafts, music sessions, outings β€” and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
