Job Description

We are seeking a Licensed Master Social Worker (LMSW) to provide social work services to patients in their homes on a per diem basis. This Road Warrior position requires travel to patient residences throughout the Arlington, TX area.

What You'll Do
  • Provide social work services to patients in their homes per physician orders
  • Develop and participate in individualized plans of care with patients and families
  • Prepare clinical and progress notes documenting patient interactions and outcomes
  • Connect families with appropriate community resources and in-service programs
  • Consult with other agency personnel to support patient care coordination
Requirements
  • Master's degree from CSWE-accredited school of social work
  • Licensed Master Social Worker in the state of Texas
  • One year experience in healthcare environment
  • Home care or hospice experience preferred
  • CPR certified with American Heart Association or American Red Cross
  • Valid Texas driver's license, auto liability insurance, and reliable transportation
  • Strong oral and written communication with excellent interpersonal skills
  • Ability to exercise independent judgment and demonstrate core agency values
Working Conditions
  • 100% travel to patient homes and agency office may be required
  • Exposure to temperature changes, odors, blood, body fluids, and hazardous materials
  • Minimal physical effort including kneeling, squatting, reaching, and minimal lifting assistance
  • Must cope with mental and emotional stress while maintaining emotional stability

Maintenance staff handle a daily work order queue β€” everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming β€” exercise classes, crafts, music sessions, outings β€” and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
