Description Setting: Skilled Nursing Facilities, Long-Term Care Communities, and/or Home Health SNF: Manor at Seagoville- Seagoville Palomino Place- Mesquite HH: Mesquite, Balch Springs, Sachse, Rowlett, Rockwall, Wylie Position Summary The Occupational Therapist (OT) provides patient-centered occupational therapy services in Skilled Nursing, Long-Term Care, and Home Health settings. The OT evaluates patients, develops and oversees individualized plans of care, and supports patients in improving independence with activities of daily living (ADLs), functional mobility, and overall quality of life in accordance with regulatory and clinical best practices. Key Responsibilities Evaluate patients and develop individualized occupational therapy plans of care Provide skilled occupational therapy treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobility Supervise and collaborate with Occupational Therapy Assistants (OTA/COTA) in accordance with state practice acts Monitor patient progress, modify plans of care as appropriate, and communicate changes to the interdisciplinary team Educate patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications Complete timely, accurate, and compliant documentation in the electronic medical record (EMR) Ensure compliance with all federal, state, payer, and facility regulations, including HIPAA Maintain a safe treatment environment and adhere to infection control and safety protocols Demonstrate professionalism, compassion, and respect in all patient and family interactions Qualifications Qualifications Graduate of an accredited Occupational Therapy program Current Occupational Therapist license in good standing (state-specific) Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply) Strong clinical, communication, and time-management skills Ability to work independently while contributing to a collaborative team environment Why Join Us Flexible part-time schedule Supportive leadership and collaborative care model Opportunity to work across multiple care settings Meaningful work making a direct impact on patient outcomes Diversity, Equity, and Inclusion are at the heart of Pivot. Infection control protocols are more rigorous than in hotels or commercial cleaning, especially during flu season or respiratory illness outbreaks.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals. Across all these roles, the common thread is that you become a familiar, trusted presence in residents' daily lives.
