Description Job Title: Field Supervisor Position Summary: Oversees care coordination and field operations to ensure safe, high-quality service delivery. Supports new client starts, reviews care expectations with caregivers, conducts supervisory visits, assists with training and orientation, participates in QAPI activities, and provides operational backup support. Care Coordination & New Client Starts: - Coordinate and support new client starts - Review plan of care, duties, and expectations with caregivers - Ensure caregivers understand documentation requirements - Document caregiver review and acknowledgment for new clients Field Supervision & Training Support: - Conduct supervisory and routine field visits - Observe caregiver performance and provide coaching - Assist with orientation and in-field training - Provide retraining when gaps are identified QAPI & Quality Oversight: - Track and report role-specific QAPI indicators - Document trends, concerns, and improvement needs - Participate in performance improvement initiatives Operational & Coverage Support: - Participate in after-hours on-call rotation - Serve as backup scheduler as assigned - Provide backup caregiving coverage when needed - Follow established chain of command Qualifications Qualifications: - Home care experience required - Caregiving experience required (CNA preferred) - Field supervision or care coordination experience preferred - Flexible availability including evenings, weekends, and on-call - Reliable transportation required

Community outreach, hospital discharge planner relationships, and local referral networks are ongoing responsibilities.
