Job Description

Join Optum Home & Community Care as a Per Assessment Nurse Practitioner serving the DFW metro area. Conduct comprehensive one-time visits for recently hospitalized patients transitioning from skilled rehab to home, capturing STAR measures and preventing readmissions.

What You'll Do
  • Perform comprehensive assessments and document findings compliant with CMS regulations
  • Ensure accurate ICD-10 coding and appropriate diagnosis documentation
  • Conduct initial medication reviews and address quality elements
  • Communicate medical concerns to patients, attending physicians, and facility staff
  • Travel between care sites to serve patients in nursing homes and senior housing
Requirements
  • Active, unrestricted Nurse Practitioner license in Texas
  • National board certification: AANP or ANCC (ANP, FNP, GNP, or AG AC NP preferred)
  • Ability to lift 30-pound bags and navigate stairs and various dwelling configurations
  • Reliable transportation and minimum 10 hours per week availability
  • Proficient computer skills with electronic medical records experience
