Job Description

Join Suvida Healthcare's Quality and Pharmacy Team as a Population Health Pharmacist serving Hispanic older adults. You'll leverage clinical expertise and data analytics to optimize medications, improve health outcomes, and support quality initiatives across our primary care program.

What You'll Do

  • Develop and implement medication optimization, adherence, and high-risk medication initiatives
  • Use payer, pharmacy, and EMR data to identify care gaps and conduct patient interventions
  • Lead quality improvement projects impacting medication-related outcomes and star ratings
  • Design and deliver medication education programs to care teams and Spanish-speaking seniors
  • Provide patient care services through collaborative practice agreements and delegated authority

Requirements

  • Doctor of Pharmacy (PharmD) from accredited institution and active Texas pharmacy license
  • Bilingual/bicultural fluency in English and Spanish
  • 1-3 years' experience in ambulatory, primary care, managed care, or population health
  • Advanced proficiency in Microsoft Office platforms (Word, Excel, PowerPoint)
  • Ability to interpret clinical data, claims data, and quality metrics to drive action

Maintenance staff handle a daily work order queue β€” everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming β€” exercise classes, crafts, music sessions, outings β€” and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
