Job Description

DFW Home Health, part of LHC Group, seeks a Registered Nurse for in-home patient care in the Denton area. Provide and direct nursing care to patients in their homes while coordinating with physicians and healthcare professionals.

What You'll Do

  • Initiate, develop, and revise plans of care in collaboration with physicians and healthcare professionals
  • Supervise home health aides and licensed practical/vocational nurses; provide instruction and assign tasks
  • Complete comprehensive assessments including OASIS and medication reconciliation accurately and timely
  • Communicate relevant information with agency staff, physicians, patients, and family members
  • Participate in on-call and weekend rotations to meet patient needs

Requirements

  • Current and unrestricted RN licensure in state of practice
  • Current driver's license, vehicle insurance, and dependable transportation access
  • Ability to function in any home situation regardless of patient circumstances
  • Ability to work independently with solid communication and organizational skills

Preferred Qualifications

  • Current CPR Certification or ability to complete within 90 days of hire
  • 1 year of Home Health experience

Compensation

  • Annual salary: $75,450 - $113,174
  • Visit point rate: $36.27 - $54.41
  • Hourly rate: $43.53 - $65.29
