Job Description
Join DFW Home Health, part of LHC Group and the Optum family of businesses, as a Registered Nurse providing in-home care. You'll deliver high-quality nursing services to patients in their homes while coordinating care with physicians and healthcare professionals.
What You'll Do
- Develop, implement, and revise patient care plans in collaboration with physicians
- Supervise home health aides and licensed practical nurses with instruction and task assignment
- Complete comprehensive assessments including OASIS evaluations and medication reconciliation
- Communicate effectively with patients, families, physicians, and agency staff regarding care coordination
- Participate in on-call and weekend rotations to meet patient care needs
Requirements
- Current and unrestricted RN licensure in your state of practice
- Valid driver's license, vehicle insurance, and dependable transportation or public transit access
- Current CPR certification or ability to complete within 90 days of hire
- Ability to work independently with strong communication and organizational skills
