Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for an RN Case Manager. $5,000 Sign-on bonus POSITION SUMMARY The Case Manager meets with patients and families to explain the scope of hospice services, coordinates admission of the patient, and develops the ongoing plan of care. ESSENTIAL FUNCTIONS Provide professional nursing care by utilizing all elements of the nursing process Assess and evaluate the patient’s status by writing and initiating a plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary Develop a care plan that establishes goals based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning process Administer medications and treatments as prescribed by the physician Provide healthcare instruction to the patient as appropriate per assessment and plan Act as Case Manager when assigned by the clinical Supervisor and assume responsibility to coordinate patient care for the assigned caseload Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition, obtain/receive physicians’ orders as needed Communicate with community health-related persons to coordinate the care plan Provide and maintain a safe environment for the patient Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver Attend interdisciplinary group meetings Participate in on-call rotation; may assist with admits Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis Supervise LVN and CNA care plans and delegate responsibilities when required. QUALIFICATIONS Registered Nurse in the state of Texas Hold a valid driver’s license and have a dependable means of transportation 1 year of nursing experience Case Management experience (hospice) 2 years of experience in hospice case management strongly preferred Proficient skills using a computer are required; Microsoft Office suite experience is preferred. What to Expect in This Role Day-to-Day

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
