Part-time Description Trio Home Health is seeking a compassionate and skilled Registered Nurse to join our team PRN / Per diem completing admissions for our new Home Health patients in the Denton, TX and surrounding areas primarily on the weekends . The primary functions of this role are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume the responsibility for coordination of care. Requirements Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. Licensure: Current state license as a Registered Nurse . Proof of current CPR certification. Current Driver's License. Experience: Two years’ experience as a Registered Nurse in a clinical setting and one-year full-time home health experience preferred but not required. Knowledge of OASIS highly preferred but not required Skills: Strong assessment and clinical skills, excellent communication skills, ability to work independently and as part of a team. Familiarity with infection control measures (OSHA). Knowledge of State and Federal regulations. Good interpersonal skills. Home Care Home Base (HCHB) knowledge highly preferred Works in patients' homes in various conditions and locations. Assess home health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs. Ability to work a flexible schedule; ability to travel locally PRN emergency call availability. Develop and document the plan of care in correlation with the assessment. Provide nursing care utilizing infection control measures (OSHA). Work with the Interdisciplinary Group to ensure patient/family needs are met. Communicate with attending physicians, home health physicians, and other physicians involved in the patient’s care. Timely submission of all required paperwork. Demonstrate commitment, professional growth, and competency. Meet productivity standards. Perform on-call responsibilities and provide on-call service to patients/families as assigned. Provide effective communication to patient/family, team members, and other healthcare professionals. Transportation: Reliable transportation and valid driver's license. Valid and current auto liability insurance. What to Expect in This Role Day-to-Day

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
