Job Description

Choice is seeking a driven, relationship-focused Home Health Business Development Liaison to expand our presence and strengthen referral partnerships across the communities we serve. This role is ideal for an experienced healthcare sales professional who thrives on building meaningful connections, growing market share, and helping patients gain access to exceptional home health services. As a key ambassador for Choice, you’ll partner with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and healthcare leaders to deliver seamless transitions of care and support outstanding patient outcomes. What You’ll Do Develop and grow referral relationships to position Choice as the preferred home health provider Build strategic partnerships with physicians, hospitals, case managers, senior living communities, and other referral sources Promote Choice’s patient-centered approach through proactive outreach and relationship management Identify and pursue new business opportunities to support market growth Provide market insight related to community needs, staffing trends, and program development opportunities Collaborate with clinical and operational leadership to expand services and enhance patient access Lead community engagement and outreach initiatives to increase awareness and strengthen local partnerships Maintain compliance with all healthcare marketing regulations and organizational standards Track referral activity, pipeline performance, and growth initiatives through CRM reporting tools Meet and exceed monthly admissions and growth goals What We’re Looking For Minimum 2 years of recent home health or hospice sales experience strongly preferred Demonstrated success developing referral networks and increasing admissions Strong relationship-building, presentation, and communication skills Experience with CRM platforms preferred HomeCare HomeBase experience preferred Valid driver’s license and current auto insurance Self-motivated, organized, and results-oriented professional Why Join Choice? What to Expect in This Role Day-to-Day

For housekeeping roles, the day follows a room-by-room schedule — cleaning resident rooms, sanitizing common areas, managing laundry, and responding to spill or accident cleanups as they happen. Infection control protocols are more rigorous than in hotels or commercial cleaning, especially during flu season or respiratory illness outbreaks.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals. Across all these roles, the common thread is that you become a familiar, trusted presence in residents' daily lives.
