Hospice PRN Field Registered Nurse (RN) – Hopestone Hospice Make every visit matter. At Hopestone Hospice, our nurses don’t just provide care—they create comfort, dignity, and meaningful moments when patients and families need it most. As a PRN Hospice Field RN , you’ll deliver compassionate, one-on-one care to patients facing life-limiting illnesses—primarily in the place they call home. You’ll have the autonomy to make clinical decisions in the field while being fully supported by a collaborative interdisciplinary team. This role is ideal for an experienced hospice RN who values flexibility, independence, and deeply meaningful work. What You’ll Do Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs Determine hospice eligibility, admission, and recertification using established clinical criteria Develop, implement, and adjust individualized plans of care to ensure comfort and quality of life Provide education and emotional support to patients and families on disease progression, end-of-life care, and self-care techniques Coordinate with the interdisciplinary group (IDG) including physicians, social workers, and chaplains Assess caregiver capability and identify support needs throughout the care journey Supervise and provide clinical direction to Hospice Aides and LVN/LPNs Document timely and accurate clinical and progress notes Communicate changes in patient condition and collaborate on care adjustments What Sets This Role Apart True autonomy in the field – practice at the top of your license Flexible PRN scheduling – pick up visits that fit your availability Meaningful, patient-centered care – smaller caseloads, deeper impact Strong team support – you’re independent, but never alone What You’ll Need Active Registered Nurse (RN) license CPR certification Valid driver’s license and auto insurance Minimum 1 year of hospice experience preferred (2 strongly preferred) Experience with Homecare Homebase is a plus Benefits & Perks 401(k) eligibility after 3 months 100% immediate vesting Company match: 100% of the first 3% contributed each pay period Why Hospice Nurses Choose Hopestone You’ll be part of a team that values compassion, clinical excellence, and the profound responsibility of end-of-life care. What to Expect in This Role Day-to-Day

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
