Job Title/Position: Licensed Practical/Vocational Nurse Reports to: Clinical Manager/Clinical Director Position Summary: The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care: Provides direct patient care as defined in State’s Name Nurse Practice Act. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager/Clinical Director. Uses equipment and supplies effectively and efficiently. Participates in personal and professional growth and development. Performs other duties as assigned by the registered nurse. Communication: Prepares clinical notes and updates the registered nurse when necessary. Communicates effectively and works closely with the office is team in a professional manner and ensures they are aware of any scheduling or patient difficulties Informs manager and/or RN of any patient care concerns or issues. POSITION QUALIFICATIONS 1. Graduate of an accredited practical nurse or vocational nursing program. 2. Has three years nursing experience and, effective January 1, 1998, Shall have one year prior professional nursing experience in Community/home health or medical/surgical experience. 3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state. 4. Complies with accepted professional standards and practice. 5. Demonstrates good verbal and written communication, and organization skills. 6. Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order. 7. Possesses and maintains current CPR certification. SKILLS REQUIRED 1. Self-directed and able to work with minimal supervision. 2. Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist. 3. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills Compliance: Acknowledge my obligation and agreement to fulfill those duties and responsibilities as set forth in the Code of Conduct and Compliance Policies and to be bound by these standards. Certify that throughout my association with EH I will comply with the terms of the Code of Conduct and Compliance Policies. Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
