Become a part of our caring community We are looking for a dedicated and experienced Primary Care Physician to join our team on a PRN (as-needed) basis. This is a zero-hour contracted role, ideal for providers looking for flexible scheduling while contributing to high-quality outpatient care. The Physician will manage a variety of health-related concerns, focusing on continuity of care, health maintenance, and disease prevention. Main Responsibilities: You will provide outpatient primary care services to adult and geriatric populations. You will maintain accurate and up-to-date medical histories and records. You will manage moderately complex to complex medical conditions. You will refer patients to specialists. You will Collaborate with interdisciplinary teams to ensure comprehensive care. You will exercise independent judgment in clinical decision-making. You will contribute to departmental strategy and quality improvement projects. We will schedule you at least 3 days a week at one of the following clinics: CenterWell Arlington, CW Grand Prairie, CW Desoto, CW Camp Bowie, or CW North Richland Hills. Use your skills to make an impact Required Qualifications: You will be a Graduate of an accredited MD or DO program. You will have an active medical license in the state of jurisdiction. You will be Board Certification in Family Medicine , Internal Medicine , or Geriatric Medicine . Preference for value-based care experience, or at least 2 years of caring for Geriatric patients with high acuities. You must be passionate about improving patient experiences and outcomes. Humana/Senior Bridge requires your participation in its Tuberculosis (TB) screening program. Preferred Qualifications: Specialty in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics. Experience working with senior populations or in value-based care models . Familiarity with electronic health records (EHR) and digital documentation. Strong communication and interpersonal skills. Ability to work independently and adapt to changing clinical environments. Additional Information: We contract this PRN role with no guaranteed hours; we schedule work based on its needs and provider availability. For housekeeping roles, the day follows a room-by-room schedule — cleaning resident rooms, sanitizing common areas, managing laundry, and responding to spill or accident cleanups as they happen. Infection control protocols are more rigorous than in hotels or commercial cleaning, especially during flu season or respiratory illness outbreaks.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals. Across all these roles, the common thread is that you become a familiar, trusted presence in residents' daily lives.
