Job Description
Join Millenia Hospice as a PRN Hospice Case Manager and provide compassionate end-of-life care in the DFW Area. This flexible role is ideal for an experienced nurse dedicated to patient-centered hospice care with dignity and clinical excellence.
What You'll Do
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care and manage symptoms effectively
- Coordinate care with physicians, social workers, chaplains, and team members
- Educate patients and families on disease progression, medications, and care techniques
- Maintain accurate clinical documentation and ensure regulatory compliance
Requirements
- Current and unrestricted RN license in Texas
- Minimum 1β2 years hospice, home health, or related experience preferred
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid driver's license, and current CPR certification
- Excellent communication and ability to work independently

Afternoons often shift to administrative work β reviewing financial reports, handling vendor relationships, preparing for state surveys, and managing marketing efforts to maintain occupancy. Community outreach, hospital discharge planner relationships, and local referral networks are ongoing responsibilities. Most directors describe the role as never having two identical days β the blend of clinical, operational, and interpersonal challenges keeps the work unpredictable but engaging.
