Job Description
We are seeking a Registered Nurse Case Manager to plan, organize, and direct home care services. The successful candidate will assess patient needs, develop care plans, and coordinate services within patients' homes and communities.
What You'll Do
- Complete initial and ongoing assessments of patients and families to determine home care needs
- Develop individualized care plans with therapeutic, preventive, and rehabilitative nursing actions
- Administer medications and treatments as prescribed by physicians
- Coordinate patient care as Case Manager and supervise home health aide services
- Communicate with physicians and community health partners to ensure coordinated care
Requirements
- Current RN licensure in your state and CPR certification
- Graduate of National League for Nursing accredited nursing school
- Valid driver's license with insured vehicle in good working order
- Minimum two years nursing experience; one year in public health or home care preferred
- Self-directed, excellent communication and problem-solving skills; ability to organize workload independently
Physical Requirements
- Prolonged walking, standing, and ability to lift and transfer patients
- Considerable reaching, stooping, bending, kneeling, and crouching
- Visual acuity and hearing to perform required nursing skills

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
