Registered Nurse - Fort Worth Area

Join DFW Home Health, part of LHC Group, as an in-home nursing professional. Provide direct patient care in homes while coordinating with physicians and healthcare teams to ensure quality outcomes.

What You'll Do
  • Develop and implement individualized nursing care plans in collaboration with physicians
  • Supervise home health aides and licensed practical nurses with instruction and task assignment
  • Complete comprehensive assessments including OASIS documentation and medication reconciliation
  • Communicate effectively with patients, families, physicians, and agency staff regarding care coordination
  • Participate in on-call and weekend rotations to meet patient needs
Requirements
  • Current and unrestricted RN licensure in state of practice
  • Current driver's license, vehicle insurance, and reliable transportation
  • Current CPR certification or ability to complete within 90 days of hire
  • One year of home health experience preferred
  • Strong communication, writing, and organizational skills
