Job Description
DFW Home Health, part of LHC Group and the Optum family of businesses, seeks a Registered Nurse for in-home patient care in the Irving area. Provide direct nursing care, supervise support staff, and coordinate comprehensive care plans with physicians and healthcare teams.
What You'll Do
- Develop, implement, and revise patient care plans with physicians and healthcare professionals
- Supervise home health aides and licensed practical nurses; assign tasks per regulations
- Conduct comprehensive assessments including OASIS and medication reconciliation
- Communicate effectively with physicians, patients, families, and agency staff about care needs
- Participate in on-call and weekend rotations to meet patient care needs
Requirements
- Current, unrestricted RN licensure in state of practice
- Valid driver's license, vehicle insurance, and reliable transportation access
- CPR certification or ability to complete within 90 days of hire
- Ability to work independently with strong communication and organizational skills
