Job Description
We're seeking a Registered Nurse (RN) for PRN/per visit home health positions in Haslet, TX. You'll manage continuity of care, conduct skilled nursing visits, and coordinate with interdisciplinary teams to support patients in their homes.
What You'll Do- Perform skilled assessments and comprehensive OASIS evaluations at patient admission
- Administer skilled nursing care including medication administration and wound care management
- Assess and chart patient condition observations, vital signs, and progress at each visit
- Maintain clinical records including visit notes and comprehensive assessments within required timeframes
- Provide patient education on home health procedures and care strategies
- Current Texas State Registered Nurse license
- 2 years of experience as an RN in home health
- Current CPR certification and TB skin test
- Valid Texas driver's license with auto liability insurance and reliable transportation
- EMR experience preferred, Kinsser experience a plus

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
