Education Master's Degree Social Work Req Experience 2 Years experience in palliative or hospice care Pref Licenses and Certifications LMSW - Licensed Master Social Worker Upon Hire Req Or LCSW - Licensed Clinical Social Worker Upon Hire Req Skills Knowledge of palliative care and hospice services, philosophy and end of life issues. Comfort in dealing with death and dying issues, complex social, spiritual and personal issues and the ability to counsel and support patients and families with dignity and respect regardless of their personal belief. Comfort with advance directive counseling and end-of-life conversations. Ability to speak to palliative and hospice philosophy and practice to advocate service line to clinical and medical staff. Ability to coordinate and collaborate with a multidisciplinary team to provide palliative and hospice care services. Knowledge of community resources including Medicare/Medicaid, financial aid, county, state and private social services agencies. Must be able to work independently, have demonstrated patient care planning and patient teaching skills and strong written and verbal communication skills. Must have general computer skills and a working knowledge of electronic heath records, MS Office, Outlook and the Internet. What You Will Do\: Assess patient/family social and emotional factors in order to estimate their potential to cope with terminal illness and death. Provide psychosocial counseling and support to the patient/family experiencing emotional, social and economic conflict. Assist patient/family and staff in utilizing community resources. Assess patient/family financial and insurance status and assist with appropriate resources as indicated. Participate in Daily Care Briefings to establish, review and revise the plan of care for each patient, including exchange of information, review of problems, of professional support. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings.
